About THE CHIME CARE DOMICILIARY PLATFORM (referred to as the "CCD" Platform")

We are a regulated domiciliary care provider and our CCD Platform is designed to connect Clients forCare Services to our approved Caregivers (Chime Care Partner).

While we are a regulated supplier of Care Services please note that our Caregivers are not directlyemployed by us. The client for Care Services will contract directly with Chime Care Ltd and Chime CareLtd sub-contracts the provision of care services to our Caregivers who are self-employed contractors.

This agreement sets out:

• who we are and what our role is
• how we support the caregivers who choose to work with us
• the terms on which you will contract with us (the 'Customer Contract') [Please see thetemplate in Schedule 1]
• the services which the care recipient will receive from our caregivers (we appreciate that theperson paying for the services, "the client", will not necessarily be the care recipient, so thisagreement distinguishes between these 2 types of people)]
• the terms on which you use our services when you use our CCD Platform

WHO ARE WE?

We are Chime Care Limited, a company registered in England and Wales under company number14611389. Our registered address is F42 Crown House, Home Gardens, Dartford, United Kingdom,DA1 1DZ. We are a subsidiary of Curamus Maxime Limited, which owns the CCD Platform which we manage and operate.

We are regulated and certified by the Care Quality Commission (‘CQC’).
